國立成功大學弱勢學生身心就醫費用補助試行要點

一、國立成功大學(以下簡稱本校)為保障弱勢學生之身心健康,適時提供相關經濟資源之協助,特訂定本要點。

1. These Directions are established by National Cheng Kung University (NCKU) to provide financial support to safeguard the healthcare of financially disadvantaged students.

 

二、本要點以學生事務處生活輔導組為業務主辦單位。

  2. These Directions shall be administered by the Student Assistance Division under the Office of Student Affairs.

 

三、補助對象:本校本國籍學生於在學期間,曾經生活輔導組核定符合低收入戶、中低收入戶、特殊境遇家庭、大專弱勢助學金或經學生事務處專案會議審查認定有其他經濟弱勢情形之當學期或次一學期,有注意力不集中、睡眠障礙、焦慮、慢性疲勞、暴食或厭食或精神衰弱等相關情形,致影響學習或生活之身心狀況,且經精神科醫生診斷為症狀明顯干擾校園及社會生活,並接受治療者。

3. Eligible applicants:

(1) Domestic students with disadvantaged family backgrounds, including low-income households, low-middle-income households, families in hardship, or financial disadvantages, who have been verified by the Student Assistance Division to qualify for subsidies.

(2) Domestic students with other financial disadvantages, verified during the current semester or the following semester and approved on a case-by-case basis at a meeting by the Office of Student Affairs, who have suffered physical and mental conditions significantly in life or academic studies, including inattention, sleep disturbance, anxiety, chronic fatigue, binge eating or anorexia, or mental weakness, and who have been diagnosed and recommended by the psychiatrist for treatment.

 

四、補助項目:扣除健保給付後實際支付之門診醫藥費用。但不包括諮商費用。

4. Subsidized items:

The applicant shall be subsidized for outpatient medical expenses actually paid with the amount of health insurance coverage deducted. However, the subsidy shall not include consultation fees.

 

五、補助金額:每人每月最高補助新臺幣1,200元(實支實付,不限門診看診次數),每人在學期間每學年最高補助6個月。

5. Amount of subsidy: A maximum subsidy of NT$1,200 per person per month (payable, unlimited outpatient visits) for a maximum of 6 months per academic year during the duration of academic studies.

 

六、申請時間:自門診看診日起6個月內,向生活輔導組提出申請。

6. Application period: A subsidy application shall be submitted to the Student Assistance Division within 6 months from the date of the outpatient consultation.

 

七、申請人應填寫申請表,並檢附下列證明文件:

(一)精神科醫生開立之診斷證明書(如為影本,請加蓋醫院或診所之戳章)。

(二)就診之醫療收據或其他足資證明醫藥費用之文件(如為影本,請加蓋醫院或診所之戳章)。

(三)學生本人之郵局存摺封面影本。

7. The applicant shall complete and submit an application form with the following documents:

(1) A diagnosis certificate issued by a psychiatrist (in case of a photocopy, it should be affixed with the stamp of the hospital or clinic).

(2) Payment receipts or other documents for medical expenses (in case of a photocopy, it should be affixed with the stamp of the hospital or clinic).

(3) A photocopy of the cover of the post office passbook.

 

 

八、本要點所需經費,由本校「學生急難救助與身心補助及其他補助」專戶項下支應。

8. The budgets required under these Directions shall be funded by the NCKU-designated account for Student Emergency Assistance, Physical and Mental Healthcare Subsidies and Other Subsidies.

 

九、本要點經學生事務會議及校務基金管理委員會通過,試行至113年7月31日止,修正時亦同。

9. These Directions shall be approved by the Student Affairs Council and the NCKU Endowment Fund Management Committee before taking effect until July 31, 2024. Amendments shall be processed accordingly.
