第一條  依據本校學則第九條訂定本辦法,學生請假依本辦法辦理。

學生缺席或曠課,扣所缺科目學業成績、補考、退學,依本校學則辦理。

全學期曠課3小時以上,扣操行成績分數,依本校學生操行成績實施辦法辦理。

Article 1.  The National Cheng Kung University (hereinafter referred to as “the University”)  promulgated the National Cheng Kung University Regulations for Student Leave of Absence (hereinafter referred to as “these Regulations”) in accordance with Article 9 of the National Cheng Kung University Academic Regulations. When taking leaves, students shall abide by said leave-taking regulations.

Grade deduction, makeup examinations, and dismissal caused by absence from class shall be handled pursuant to the National Cheng Kung University Academic Regulations.

Deduction of personal conduct grade caused by absence of 3 hours or more shall be handled pursuant to the National Cheng Kung University Regulations Governing Personal Conduct Grading.

 

第二條  學生請假,分為病假、事假、公假、喪假、產假(含產前、分娩、哺育及陪產假)、生理假及器官捐贈假(含骨髓、器官)。

Article 2.  Student leaves are categorized into sick leave, personal leave, official leave, bereavement leave, maternity leave (including prenatal care, postnatal care, baby nurturing, and paternity leave), menstrual leave, and organ donor leave (including organ and marrow bone donation).

 

第三條  學生請假除病假外,應事先提出申請。

因重病或臨時發生緊急、重大不可抗拒之變故,應於請假最後一天次日起算五天內提出請假申請。但應先以電話、書信或委託他人等方式向授課老師報備;若因故無法與授課老師取得聯絡者,亦得向導師或系主任(所長或學位學程主任)報備。

學生未依規定請假,或請假未准而缺席者,以曠課論。

Article 3.  With the exception of sick leave, students shall submit a leave application in advance.

Students affected by serious illness, emergency, or unforeseen contingencies shall submit a leave application within 5 days after the last day of the leave. Nevertheless, the student concerned shall notify their instructor(s) by phone call or written notice or by assigning others to pass on the message regarding their absence from class. Should the student fail to contact their instructor(s), they shall report their circumstances to the advisor or the department chair (or director of the graduate institute or the program).

Those who fail to complete their leave application according to the aforementioned Regulations or who fail to receive approval for their leave application shall be considered absent from class.

 

第四條  病假、事假、喪假、產假(含產前、分娩、哺育及陪產假)、生理假、器官捐贈假(含骨髓、器官)之請假核准程序如下:

一、請假日數二天以內:由授課老師核准。

二、請假日數三天(含)以上:由授課老師及系主任(所長或學位學程主任)核准。

公假之申請,由系主任(所長或學位學程主任)核准。因公派遣學生出任公務之行政單位主管核准後,應向系主任(所長或學位學程主任)報備。

生理假提出申請後,不適用前二項之核准程序,逕送學生事務處生活輔導組備查。

學生請假核准後,送學生事務處生活輔導組備查。

Article 4. Applications for sick leave, personal leave, bereavement leave, maternity leave (including prenatal care, postnatal care, baby nurturing, and paternity leave), menstrual leave, and leave for organ donation (including bone marrow or organ) shall be processed as follows:

1. Leaves of no more than 2 days: Applications shall be approved by the course instructor(s).

2. Leaves of 3 days or more: Applications shall be approved by the course instructor(s) and the department chair (or director of the graduate institute or the program).

Applications for official leave shall be approved by the department chair (or director of the graduate institute or the program). Official leave applications from students assigned by an administrative unit to perform public duties shall be approved by the chief of the unit, and the department chair (or director of the graduate institute or the program) shall be informed of the leave.

Applications for menstrual leave, which are exempted from approval procedures prescribed in Rules 1 and 2, shall be directly submitted to the Student Assistance Division of the Office of Student Affairs and be filed for future reference.

All leave applications approved shall be submitted to the Student Assistance Division of the Office of Student Affairs and be filed for future reference.

 

第五條  學期考試期間不得請假。但因重病,或臨時發生緊急、重大不可抗拒之變故,應檢附相關證明文件,並依本辦法第三條規定辦理。

前項但書學期考試之請假,由授課老師及系主任(所長或學位學程主任)核准。

Article 5. Leave-taking during semester examinations is prohibited. However, in case of severe illness, emergency, or unforeseen contingencies, students are allowed to apply for a leave with relevant supporting documents in accordance with Article 3 of these Regulations.

Applications for leave during semester examinations shall be approved by course instructors and the department chair (or director of the graduate institute or the program).

 

第六條  學生請假應檢附文件如下:

一、病假:請假三天以上,須檢具醫療院所之證明。

二、事假:請假三天以上,須有家長、監護人文件或其他足資證明之文件。

三、公假:

(一)經選派代表學校參加校內或校外活動,有相關單位出具證明文件者。

(二)經選派擔任公務活動,有單位主管出具證明文件者。

(三)有關兵役事項,有兵役單位出具證明文件者。

(四)基於法定義務出席作證、答辯,有相關單位出具證明文件者。

(五)參加政府依法主辦之考試、訓練,有相關證明文件者。

(六)其他依法規定應給公假者。

四、喪假:

(一)學生之配偶,或學生及配偶之直系親屬、兄弟姊妹或以共同生活為目的之同居人喪葬,以十天為限;其他親屬喪葬以一天為限。

(二)須檢具訃文或死亡證明書(影本可);共同居住者須檢具戶口名簿或戶籍謄本(影本可)。

五、產假(含產前、分娩、哺育及陪產假):

學生因懷孕引發之事(病)、生產及哺育三歲以下幼兒或配偶陪產,得持醫療院所或相關證明辦理請假;陪產假以七天為限。

六、生理假:因生理期不適而請假者,每月以一天為限,無須檢具證明。

七、器官捐贈假(含骨髓、器官):捐贈骨髓或器官者,須檢具醫療院所相關證明辦理,以一個月為限。

Article 6.  Applications for leave shall be submitted with relevant documents as follows:

1. Sick leave for 3 days or more: Applicants are required to submit a medical certificate or document issued by the hospital or clinic.

2. Personal leave for 3 days or more: Applicants are required to submit documents indicating the approval of their parents or legal guardian or relevant supporting certificates.

3. Official leave:

(1) Applicants assigned to represent the University to attend intramural or extramural activities are required to submit relevant documents issued by the unit(s) concerned.

(2) Applicants assigned to engage in official businesses are required to submit relevant documents issued by the supervisor of the unit concerned.

(3) Applicants involved in military services are required to submit the certificates issued by the military service unit.

(4) Applicants legally obligated to testify or make pleas are required to submit certificates issued by the relevant unit(s).

(5) Applicants attending examinations or training programs organized by the government pursuant to laws are required to submit relevant certificates.

(6) Applicants granted other types of official leave pursuant to laws are required to submit supporting certificates.

4. Bereavement leave:

(1) Students may submit an application for funeral leave for a maximum of 10 days due to the passing away or funeral service of their spouse, lineal relative(s), sibling(s), or cohabitant(s) living in the same household. For bereavement leave due to passing away or funeral services of other relative(s), students may submit an application for funeral leave for a maximum of 1 day.

(2) Applicants are required to submit an obituary or a certificate of death (photocopies are permitted). For a cohabitant, a household certificate or a household registration transcript (photocopies are permitted) shall be submitted in addition to an obituary or a death certificate.

5. Maternity leave (including prenatal care, postnatal care, baby nurturing, and paternity leave):

Applications for personal (sick) leave due to pregnancy, delivery, and care of children under the age of three years and for paternity leave shall be submitted with certificates issued by the medical institution or clinic concerned. Students may apply for a paternity leave for a maximum of 7 days.

6. Menstrual leave:

Students may apply for a menstrual leave for a maximum of 1 day per month. No certificate is required.

7. Organ donor leave:

Students may apply for a leave for bone marrow or organ donation for a maximum of 1 month with certificates issued by the medical institution or clinic concerned.

 

第七條  本辦法經學生事務會議通過並陳校長核定後實施,修正時亦同。

Article 7. These Regulations and subsequent amendments come into effect upon approval by the Student Affairs Council and ratification by the President of the University.
