第一條

對於罹患重大疾病、遭逢意外傷害或家庭變故之本校在學學生，為使其及其家庭獲得適時扶助，並協助其順利完成學業，特訂定「國立成功大學學生緊急紓困及家庭急難慰助金實施辦法」。（以下簡稱本辦法）

Article 1

For students who suffer from major illnesses and accidental injuries or students whose families encounter major changes, the “National Cheng Kung University Regulations for Emergency and Condolence Allowances” (hereinafter referred to as the Regulations) are established to offer students and their families timely assistance and help them complete studies successfully.

第二條

經費來源：由本校學雜費收入之獎助學金提撥款辦理。

Article 2

Funding source: Scholarship from tuition and fees.

第三條

學生發生意外事故或傷病，申請標準與核發金額如下：

(1)     不幸亡故者，核發新台幣參萬元整。

(2)     重傷或重病就醫者（診斷證明住院一週以上），核發新台幣壹萬元。

(3)     學生符合全民健保重大傷病標準者，核發新台幣壹萬元。

(4)     學生家庭遭逢重大變故者（例如風災、火災、水災、震災及法定災害等財物嚴重損失者） ，致使生活陷入困境，嚴重影響就學，核發金額如下：

1.    財務嚴重損失者（持消防機關、警政或鄉鎮區公所證明文件），核發新台幣伍仟元整（僑外生檢具駐外單位認可之證明文件） 。

2.    房屋半毀（倒）者（持鄉鎮區公所證明文件）核發新台幣壹萬元整（僑外生檢具駐外單位認可之證明文件） 。

3.    房屋全毀（倒）者（持鄉鎮區公所證明文件），核發新台幣貳萬元整（僑外生檢具駐外單位認可之證明文件） 。

4.    私有田地遭流失者（持鄉鎮區公所證明文件），核發新台幣壹萬元整（僑外生檢具駐外單位認可之證明文件） 。

(5)     配偶、子女因傷病而死亡者：核發新台幣貳萬元整。

(6)     父親、母親符合一方死亡者：核發新台幣貳萬元整。

(7)     父母雙方死亡者，核發新台幣伍萬元整。

Article 3

For students encounter accidents or suffer from illnesses or injuries, the application criteria and the amount of allowance issued are as below:

(1)       For deceased students, NTD$ 30,000 shall be issued

(2)     For students who are hospitalized due to serious illnesses or injuries (certificate of diagnosis shown students are hospitalized for more than one week), NTD$ 10,000 shall be issued.

(3)     For students who meet the requirement of catastrophic illness of National Health Insurance, NTD$ 10,000 shall be issued.

(4)     Families of students encountering major changes: For example, families suffer from typhoons, fires, floods, earthquakes or any legal disasters which cause their lives from falling into difficulties. The amounts of allowances are as below:

1.          Students suffer from huge financial losses shall provide documents from the Fire Agency, Police Agency or Township (City) Office. NTD$ 5,000 shall be issued (Overseas Chinese students and international students shall provide documents approved by Ministry of Foreign Affairs' overseas embassies).

2.    For houses with major damaged, students shall provide documents from the Township (City) Office. NTD$ 10,000 shall be issued (Overseas Chinese students and international students shall provide documents approved by Ministry of Foreign Affairs' overseas embassies).

3.    For destroyed houses, students shall provide documents from the Township (City) Office. NTD$ 20,000 shall be issued (Overseas Chinese students and international students shall provide documents approved by Ministry of Foreign Affairs' overseas embassies).

4.    Students deprived of their private fields shall provide documents from the Township (City) Office. NTD$ 10,000 shall be issued (Overseas Chinese students and international students shall provide documents approved by Ministry of Foreign Affairs’ overseas embassies).

(5)     For students whose spouse or children died of illnesses or injuries, NTD$ 20,000 shall be issued.

(6)     For students whose father or mother deceased, NTD$ 20,000 shall be issued.

(7)     For students whose father and mother deceased, NTD$ 50,000 shall be issued.

第四條

申請紓困急難慰助金，除檢附學生證正、反面影本外，尚須依申請之原因事實分別檢具下列相關文件：

(1)  不幸亡故者：附死亡證明書。

(2)  重傷或重病就醫者：附診斷證明書（須住院一週以上）。

(3)  學生符合全民健保重大傷病標準者：附健保局核發之重大傷病卡。

(4)  學生家庭遭受重大變故者：附所屬鄉鎮市區公所證明文件（僑外生檢具駐外單位認可之證明文件）。

(5)  配偶、子女因傷病而死亡者：附死亡證明書及戶籍謄本正本（僑外生需附與申請者關係之相關證明文件）。

(6)  父親、母親符合一方死亡者：附死亡證明書及戶籍謄本正本（僑外生需附與申請者關係之相關證明文件）。

(7)  父母雙方死亡者: 附死亡證明書及戶籍謄本正本（僑外生需附與申請者關係之相關證明文件）。

Article 4

Students who apply for emergency and condolence allowances shall provide a copy of

Student ID card (front and reverse side) along with the following documents according to different facts.

(1)       Deceased: Provide death certificate

(2)     Students hospitalized due to serious illnesses or injuries: Provide certificate of diagnosis (hospitalized for more than one week).

(3)     Students who meet the requirement of catastrophic illness of National Health Insurance: Provide catastrophic illness card issued by the Bureau of National Health Insurance.

(4)     Families of students encountering major changes: Provide documents from the Township (City) Office (Overseas Chinese students and international students shall provide documents approved by Ministry of Foreign Affairs’ overseas embassies)

(5)     Spouse or children died of illnesses or injuries: Provide death certificate and the original copy of certificate of household registration (Overseas Chinese students and international students shall provide documents showing the relationship to the applicant)

(6)     Father or mother deceased: Provide death certificate and the original copy of certificate of household registration (Overseas Chinese students and international students shall provide documents showing the relationship to the applicant)

(7)     Both father and mother deceased: Provide death certificate and the original copy of certificate of household registration (Overseas Chinese students and international students shall provide documents showing the relationship to the applicant)

第五條

本辦法之申請期限及核發程序如下：

(1)     本校各系所或教官發現有同學符合本辦法申請標準時，應由該生或教官填寫本校「學生緊急紓困及家庭急難慰助金申請表」，轉由導師向學務處軍訓室提出申請。

(2)     申請期間，應於各項事故發生之三個月內提出申請，並檢附該生在學及相關證明文件，向學務處軍訓室提出申請。經學務處複審後，依行政程序送交相關單位審核，經核准核發之慰助金，直接匯入受慰助者本人帳戶或由出納組開據支票轉發受慰助者本人或家長。

(3)     同一事件之申請，以一次為限。

Article 5

The application period and issue procedure of the Regulation are as below:

(1)       If the Department/Institution or military instructor finds that a student meets the application criteria of the Regulation, the student or the military instructor shall fill out the “National Cheng Kung University Regulations for Emergency and Condolence Allowances Application Form”. Then, the advisor forwards the application to the Military Training Division of the Office of Student Affairs.

(2)     The application along with the certificate of enrollment and relevant supporting documents shall be filed within three months of the accident to the Military Training Division of the Office of Student Affairs. After the secondary review conducted by the Office of Student Affairs, the application is forwarded to the relevant unit for review according to administrative procedures. The approved allowance will be directly transferred to the recipient’s account. Or a check will be forwarded by the Cashier Division to the recipient or his or her parents.

(3)     One event is limited to one application.

第六條

本辦法經學生事務會議通過並陳校長核定後實施，修正時亦同。

Article 6

The Regulations is approved by the meeting of the Student Affairs Council and President before taking effect. Any amendments shall be processed accordingly.
