一、依據:教育部補助大專校院招收及輔導身心障礙學生實施要點經常門經費補助基準項目三。(104 四、審核程序:本要點之審核以一學期為限,審核申請表如附件一,審核 五、審核小組: 1. These guidelines are formulated in accordance with current account item (3) specified in the Ministry of Education Directions of Recruiting and Counselling Students with Physical and Mental Disabilities. (code of the official document: Tai Jiao Xue (iv) no. 1040102445B) 2. Objective: To provide transportation subsidy for students of the university who have a certificate of disability and are unable to commute to and from school. 3. Requirements: (The following 3 criteria shall be met.) (1) The student who is registered as students of the university in the current semester and has a certificate of physical and mental disability. (2) The types and severity of physical and mental disability make it impossible for students to commute to and from school on his/her own. (3) The student does not live on campus. 4. Examination procedure: The validity period of this examination is one semester. The application form is shown in Annex 1 and the examination procedure is shown in the following figure. 5. Examination team: (1) The examination team consists of five members, the Director of Counseling and Wellness Services Division, three counselors of special education in the resource classroom, and a professional who is off-campus physical therapist, occupational therapy or physician of the related field. (2) The attendance fees for professionals are paid according to the standards listed in the Ministry of Education subsidies and the commission program, and are recorded under the current account of “Work Plan for Counseling Disabled Students” of the university. 6. Principles of examination: (1) The student is restricted by his/her mental and physical disability and are not able to use similar means of transportation as other students. (2) The disability of the student can affect his/her ability to commute to school. For example, lower limb disorders, severe epilepsy, total blindness, cerebral palsy, muscular dystrophy or other rare diseases. (3) The student does not live on campus. 7. Application materials: (1) Photocopy of valid proof of physical and mental disability (2) Photocopy of valid certificate of authentication issued by the Committee Responsible for Identification and Placement of Gifted and Disabled Students (3) Application form for transportation subsidy 8. Amount of subsidy: Each student may receive a transportation subsidy of NT$800 per month based on the actual number of months in a semester, up to 9 months per year. This amount is subject to annual subsidy provided by the Ministry of Education for this project. (These regulations were translated from the original Chinese.
